Ravitch modificata senza cicatrice anteriore
Ho sentito molto parlare di questa tecnica e ne vorrei sapere di piu`. Fra pochi mesi mi operero` con la tecnica Ravitch classica. Qualcuno ha subito la nuova tecnica? In cosa consiste? Perche` ha preso poco piede in Italia (propopsta solo in centri come Milano)? E come mai viene presa in considerazione solo in pochi casi? Magari se un esperto chirurgo che partecipa a questo forum potesse cortesemente illuminarci ne sarei veramente grato!!!

Abstract
We began in 1982 to use a modified Ravitch procedure that preserves the blood supply to the sternum to correct funnel chest deformities, but there were some problems such as postoperative paradoxical respiration, chest wall irregularity, and palpable heart beat. To resolve these problems, the concept of Jensen's procedure was incorporated into the authors' previous method. In Jensen's method, only a small portion of the deformed cartilages is resected, and almost all of them are preserved. A preformed stainless steel strut is used for chest wall stabilization. The authors preserved the costal cartilages in a manner similar to Jensen's, but the anterior chest wall was stabilized with miniplates and Kirschner wires instead of a large retrosternal strut. The xiphoid process with the lower end of the sternum was detached and moved cephalad. None of the patients developed respiratory failure postoperatively. Although the follow-up periods were short, satisfactory results were obtained with no recurrence of the deformities. The main advantages of our procedure are (1) basic blood supply to the chest wall is preserved, (2) stability of corrected chest wall is maintained with miniplates and Kirschner wires without a large metallic strut, and (3) preserved costal cartilages make the chest wall rigid and the incidence of respiratory failure low. We conclude that this method is simple and produces satisfactory results with a rigid chest wall.

- perchè ha preso poco piede in Italia ? : forse perchè i poveri chirurghi Italiani non la considerano efficace !!!
- la tecnica non è eseguita in pochi centri in Italia .... come Milano, ma bensì ( questo a me risulta ) è eseguita in 1 solo centro e questo centro è a Milano ! ( sono due cose diverse ....... )
- è una tecnica che tutti i chirurghi possono usare, non è segreta e tantomeno coperta da copyright ; io non la uso perchè la ritengo, come tanti altri appunto , non valida !!! ............ ma questa è una mia opinione.

mi viene da pensera che con le incisioni laterali..non si riesca a raggiugere un risultato ottimale in quanto non c'è la visione completa delle strutture cartilaginie....il compromesso io penso che sia solo uno......operarisi o non farsi operare.......ravicht con taglio verticale o niente..le cose fatte a meta' non vanno mai bene.....presumiamo che farai quella con i tagli ai lati e non rimani soddisfatto cosa fai????poi ancora in quel centro che dici te la baretta rimane a vita..........rilfettici bene........

Oggi Dottore .... per gli interisti non e' un gran giorno.... Di vaio ha fatto le 2 incisioni laterali e Acqua fresca ha messo la barra alla fine
Per quanto concerne la Ravitch modificata so solo che la fa solo un Chirurgo per i risultati trovi su internet le risposte alle tue domande....
